3. Once you have downloaded it, you can save it to your disk and open it or unzip it depending which file you have chosen.
4. Fill in the form as instructed.
5. Once completed, save it in a file in your computer. Then e-mail us your order form file by attaching it as an attachment file in your e mail. We are flexible. If you prefer the non E method, just fax to us.
